The purpose of this Notice of Funding Opportunity (NOFO) is to solicit UG3/UH3 phased, cooperative agreement research applications to evaluate, implement, and/or test population-based interventions to policies, programs, and/or practices for promoting integration of services, systems, and/or values by leveraging cross-sector partnerships in communities. Such interventions must address social and chronic disease common risk factors and/or barriers driving oral disease burden in communities. Projects funded through this funding opportunity and a separately funded Community Engagement Evaluation and Data Coordination hub (CEED) will form a Community-Based Participatory Research (CBPR) consortiu
